ABSTRACT


The purpose of the study is to clarify the effects of thermal stimulation on the autonomic nervous system and the cardiovascular system using spectral analysis of the variability of heart rate and arterial pressure.In 15 healthy subjects, arterial pressure, heart rate nad power spectral density of these parameters were measured during the immersion of the right hand in cold and warm water (at 5, 34, 48℃) for 2 minutes. Each parameter was unchanged in the immersion in the water at 34℃. In contrast, in the immersion in the water at 5 and 48℃, the high-frequency (0.15-0.40 Hz) power spectral density (HF)(mesc2) of the heart rate variability was decreased, but the ratio of the low-frequency (0.04-0.15Hz) power spectral density (msec2) to HF was increased. In parallel with these changes, the low-frequency(0.02-0.15Hz) power spectral den-sity (mmHg2) of the systolic arterial blood pressure was increased. The arterial blood pressure and the heart rate were also increased by the thermal stimulation.
The mechanism of these responses was considered to be mediated through the somatosympathetic reflex, because the responses of these parameters occurred immediately after the immersion of the right hand in both the cold and warm water. These results indicate that thermal stimulation affects the cardiovascular system through the modulation of the autonomic nervous system in humans. Thus, the change in the environmental temperature might be one of the factors which cause myocardial infarction and sudden cardiac death.



 要 旨

 心臓突然死を含む心事故の発生には自律神経の変調が関与しており、また、心事故の発生には環境温度の変化が関与していることが、疫学的に知られている。そこで、急激に温度が変化した時の心臓・血管系の反応とその機序を知るために、ヒトの皮膚の一部に温度刺激を与え循環器および自律神経系の機能を非観血的、経時的に測定した。その結果に基づいて、温度変化と心事故発生の関係を考察した。
 健常人15名を対象に、右手を2分間、5℃、34℃(無関温度)、48℃の3種類の冷水や温水に浸け、心電図と非観血的に対側の血圧を記録した。最大エントロピー法による心電図R−R間隔と血圧のピーク値変動の周波数解析により、自律神経機能を評価した。34℃では、温度負荷により測定値に変化は見られなかった。低温および高温刺激により、交感神経機能は亢進し、副交感神経機能は低下し、心拍数と血圧は増加した。これらの変化の反応機序は、皮膚温度受容器の興奮が体性神経を経て脊髄と循環中枢に伝達され、心臓・血管自律神経を介して心臓、血管に作用する、体性−自律神経反射を介するものと考えられた。
 本研究により、ヒトでは、低温、高温の温度変化が交感神経系を介して心臓・血管系の機能を亢進させることを明らかにしたと考えられる。諸家の報告と合わせて考えると、環境温度変化が交感神経機能を亢進し、心事故の発生に関与することを示唆している。
